After you selected the service you wish for Maritime Funeral Services to perform, you will need to follow these few steps below to complete the arrangement.
An Authorization will need to be prearranged in order for Maritime Funeral Services to perform your preferred service. Maritime Funeral Services Authorization Form (MFSAuthForm can be downloaded below) is designed to be printed from your browser, however we will be happy to mail you the form if you wish. Please Contact Us if you would like the form mailed or faxed.
Obtain a copy of the death certificate. This is a requirement of the United States Coast Guard as well as the U.S Environmental Protection Agency.
Obtain a copy of the Release form from the crematory (this may be sent by the crematory with the remains).
Take the above 3 forms along with the cremated remains to the post office. The US Postal Service will ship cremated remains however you will NOT be able to use FedEx, UPS or DHL. You will want to use US Priority mail with a certified receipt requested. The cremated remains must remain in the plastic/cardboard box in which you received them. Place the original box inside another new box. Insert the completed forms as indicated in steps 1-3 as well as a bank cashier's check if have not already paid by Paypal or credit card.
The Family Scattering Service can be arranged to meet your family's needs. You may choose up to six people to attend, including a member of the clergy if you wish.
